Our south side office is located at 5663 South 27th Street within a medical clinic. This location is convenient to the airport and the highway. If traveling from the north/west or use exit 318 (27th Street) off of 894/43 or if traveling from the south, use exit 319 (College Ave.) off of I-94. In addition, we have teamed with Bryant Masencarenhas and Santosh Yoga to offer Beginner Yoga classes at 7:30 pm on Thursday evenings. These provide an excellent fitness opportunity to patients of all ages and fitness abilities.

Frank Fantazzi P.T. Frank Fantazzi, PT, OCS, is the co-owner and founder of PT Plus. Following nearly 30 years in the field of physical therapy, he continues to find time to focus his energy on his clinical practice as well as mentor and coach physical therapists who are looking to open their open private practices. Frank’s distinguished career began in 1977 when he graduated from Marquette University. His early career included working at Wood Veteran’s Administration Hospital in Milwaukee and outpatient orthopedics. He was an adjunct clinical instructor for Marquette University from 1982-1989, teaching orthopedic evaluation and treatment. In 1983, Frank entered into private practice. By owning and managing Associated Physical Therapists of Milwaukee, Frank discovered his mission within physical therapy is private practice. He and his wife Maureen successfully grew their business to 7 clinics across Southeastern Wisconsin. Frank’s training in manual therapy and orthopedics spanned his career. He has trained with pioneer’s in the field including Robin McKenzie, Stanley Paris, John Upledger, Phil Greenman, Fred Mitchell, Laurence Jones and John Barnes to name a few. In 1990 he received his certificate in manual therapy from The Institute for Graduate Health Sciences and in 1991 received his Orthopedic Certified Specialist from the American Physical Therapy Association. In addition, he underwent specialty training including Key Functional Capacity Assessment, TMJ or Temporomandibular Joint Dysfunction, Facial Paralysis Rehabilitation, Craniofacial pain, and Chronic Pain Treatment and Management. Frank continues to be an active member of the American Physical Therapy Association and Private Practice Section as well as the Wisconsin Physical Therapy Association. He continues to be an advocate for advancing the knowledge and expertise of physical therapists. Most recently he has completed training with John Iams, PT and SuperSpine Inc. to be an Instructor for the Basic Training in Primal Reflex Release Technique™. Frank and his wife Maureen have lived in Brookfield since 1983. They have raised 4 wonderful children. Amy Snyder, MPT Amy Snyder, MPT, is a graduate of the University of Florida. Following graduation, Amy spent one year training with the National Centers for Facial Paralysis in Washington D.C., making her a specialist in the field of craniofacial pain and dysfunction. Following that time, Amy returned to her home town of Milwaukee to work in private practice with her parents and husband. Since then they have expanded their business to four PT Plus locations- Racine, Elmbrook, downtown Milwaukee, and the southside of Milwaukee. Amy has focused her continuing education on enhancing her manual therapy skills. She has attended numerous seminars. Her varied education includes Myofascial Release, Muscle Energy Technique, Strain-Counter Strain, Cranial-Sacral therapy, and Primal Reflex Release Technique. In addition, Amy is studying with Creighton University to complete her Doctorate in Physical Therapy. Her expected graduation is in December of 2007. She is a member of the American Physical Therapy Association and the Wisconsin Physical Therapy Association. In her spare time she enjoys practicing Yoga and Pilates and incorporates these exercise philosophies into her daily treatments to enhance her patients overall health. “I believe in treating the whole person. Mark Snyder, P.T. Mark is an owner/partner in the Elmbrook and 3rd Ward PT Plus offices. He is a 1992 graduated of the University of Maryland. He has worked in acute care, sports medicine, and outpatient orthopedics. Mark has been a director in a sports medicine clinic, a private practice owner as well as a vice president and director of field operations at the National Centers of Facial Paralysis in Washington, DC. With a strong passion for education, Mark has been a professor for a physical therapy assistant program and has taught continuing education to physical therapists, occupational therapists, massage therapists and physicians in the US and in Europe. He has also taken over 50 continuing education classes in areas such as myofascial release, joint mobilization, strain/counterstrain, muscle energy technique, cranio-sacral, visceral mobilization, lymph drainage, neural tissue tension technique, biofeedback, motor nerve conduction velocity testing, and Primal Reflex Release Technique (PRRT®). Currently, Mark is enrolled at Creighton University studying for his Doctorate in Physical Therapy. Maureen Fantazzi P.T. Maureen graduated from Marquette University in 1975. She began her career working with geriatric populations in Ohio and Wisconsin. In 1980 she began her own business, contracting to provide physical therapy services to extended care facilities in the Milwaukee area. She continued until 1987, at which time she joined forces with her husband, Frank Fantazzi, to open their own private practice. Their company, Associated Physical Therapists of Milwaukee specialized in providing outpatient orthopedic services. Together they grew this business to include 7 offices across southeastern Wisconsin. During that time Maureen re-focused her career goals and became more involved in business management for their growing company. She has attended numerous continuing education classes geared toward coding, billing, re-imbursement and business management for the physical therapist. Maureen and her husband eventually sold APTM and began their latest venture, PT Plus. Maureen currently provides business management at our Elmbrook and Milwaukee offices. Bryant Mascarenhas has been studying Iyengar Yoga with Rev. Father Joe Pereira since 1986. He has recently returned from India where he taught over 250 practioners in his home city of Bombay. He had a unique private session with global icon, B.K.S. Iyengar and his mentor, Fr. Joe Pereira. Bryant teaches, lives and breathes yoga full-time. Yoga is the best exercise system developed to integrate your physical health, emotional stability and your spiritual life. Yoga can be learned and practiced by all ages from as young as 6 to as old at 91.
